The symptoms of an injury to your Achilles tendon depend on whether the tendon is irritated, strained, partially torn, or fully ruptured. Early Achilles problems may begin as soreness or stiffness at the back of the ankle or just above the heel. Pain may be worse after running, jumping, sprinting, climbing stairs, or increasing training intensity.

Advanced imaging may also be recommended to evaluate the tendon more clearly. Depending on the injury, this may include X-rays to assess nearby bone structures, ultrasound to evaluate tendon continuity and movement, or MRI to define the size, location, and severity of a tear. This diagnostic process helps distinguish Achilles tendonitis from tendinosis, partial tearing, or a complete Achilles tendon rupture. During the examination, your physician may also perform simple clinical tests, such as the Thompson test, to evaluate whether the Achilles tendon remains intact and determine whether a rupture is likely.

Whenever appropriate, SCOI begins with nonsurgical treatment before considering surgery. Treatment recommendations are individualized based on the specific injury, activity level, tendon quality, overall health, and patient goals. Many mild to moderate Achilles tendon injuries improve without surgery. Conservative treatment may include rest, ice, compression, elevation, anti-inflammatory medication when appropriate, supportive footwear, heel lifts, orthotics, and physical therapy. Physical therapy may focus on calf flexibility, tendon loading, ankle mobility, gait mechanics, and progressive strengthening.
For persistent Achilles tendonitis, tendinosis, or partial injury patterns, SCOI specialists may recommend more structured rehabilitation, bracing, boot immobilization, or image-guided care when appropriate. The goal is to reduce pain, protect healing tissue, and restore tendon strength without rushing a return to activity.
Surgery may be considered for a torn Achilles tendon injury, complete rupture, severe tendon damage, or pain that does not improve after an appropriate course of nonsurgical care. Depending on the condition, treatment may involve tendon repair, tendon transfer, or procedures designed to reduce stress on the Achilles. SCOI physicians help patients understand when surgery is appropriate and when nonsurgical care may be a better fit.
Recovery from an Achilles tendon injury depends on severity. Tendonitis and strains may improve with activity modification and rehabilitation, while partial tears and ruptures require longer protection and a more structured return-to-activity plan.
Recovery is typically gradual rather than linear. Most patients progress through phases of protection, strengthening, balance training, and functional rehabilitation before returning to unrestricted daily activities or sports.
