Soccer injuries most commonly affect the knees, ankles, and lower-body muscles due to cutting, sprinting, and contact during play. Common injuries include ACL and MCL tears, meniscus injuries, ankle sprains, hamstring strains, and concussions. Early symptoms such as swelling, instability, or pain with movement should not be ignored, as prompt evaluation helps prevent more serious injury and supports a safer return to play.

Soccer injuries occur during matches and training due to the sport’s demands on speed, cutting, kicking, and physical contact. Players sprint, plant, and change direction, and strike the ball with force. These movements place significant stress on the lower body. Studies of youth and professional leagues show that most bad soccer injuries affect the knee, ankle, or surrounding muscles, with rapid acceleration, sudden deceleration, and player contact increasing the risk.

Sports medicine physicians diagnose soccer injuries through a focused history, physical exam, and imaging, such as X-rays or MRI when needed. They then create individualized treatment and rehabilitation plans, guiding athletes through progressive strengthening and sport-specific drills to ensure a safe return to play and reduce the risk of reinjury.

Most Common Soccer Injuries

Sports medicine physicians often group soccer sports injuries by body region because the sport’s movements place repeated stress on specific joints and muscle groups, most commonly the knee, ankle, and sprinting muscles.

Knee Injuries: Cutting, Pivoting, and Landing

The knee absorbs enormous force during soccer. Injuries frequently occur when players plant their foot to cut past a defender or land awkwardly after going up for a header.

Knee injuries are among the most serious in soccer and often require structured rehabilitation or surgical evaluation to ensure full recovery.

ACL Tears: The anterior cruciate ligament (ACL) stabilizes the knee during rotation and is often injured when players plant and pivot or change direction at full speed. Symptoms include:

  • Sudden pop in the knee
  • Immediate swelling
  • Instability when pivoting or cutting
  • Difficulty continuing play

MCL Sprains: The medial collateral ligament (MCL) stabilizes the inner knee and is commonly injured when contact forces the knee inward during tackles, limiting lateral movement and stability. Symptoms include:

  • Pain along the inner knee
  • Swelling
  • Difficulty cutting or turning

Meniscus Tears: The meniscus cushions the knee joint and can tear during twisting movements while trapping the ball or changing direction, often making pivoting or planting painful during play. Symptoms include:

  • Knee pain or catching
  • Swelling after activity
  • Difficulty fully straightening or bending the knee
  • Knee becoming stuck or locked

Patellofemoral Pain Syndrome (Runner’s Knee): Repetitive running and kicking can irritate the joint behind the kneecap, often becoming noticeable during longer training sessions or matches. Symptoms include:

  • Dull pain around the kneecap
  • Pain when climbing stairs or squatting
  • Discomfort after training sessions

Ankle & Foot Injuries: Quick Cuts and Hard Landings

The ankle is one of the most commonly injured joints in soccer due to rapid direction changes and player contact.

Lateral Ankle Sprains: These injuries often occur when players roll the ankle during a cut or land on another player’s foot, causing instability that can affect sprinting and directional changes. Symptoms include:

  • Swelling around the ankle
  • Pain when walking or pushing off
  • Limited range of motion

Achilles Tendinitis: Repetitive sprinting and explosive acceleration place stress on the Achilles tendon, making pushing off difficult. Symptoms include:

  • Tightness in the back of the ankle
  • Pain during warm-ups or after matches

Turf Toe: This sprain of the big toe joint occurs when the toe bends upward during push-off, affecting acceleration and balance during play. Symptoms include:

  • Pain at the base of the big toe
  • Difficulty pushing off while running

Foot Fractures (Metatarsal Stress Fractures): Stress fractures can develop from repeated impact during training or competition, causing gradual, localized pain on the top or ball of the foot that worsens with weight-bearing. Symptoms include:

  • Tenderness when touching the bone
  • Swelling
  • Potential bruising
  • Limping gait

Footballer’s Ankle: Repeated kicking, running, and squatting can lead to bone spurs and chronic pain at the front of the ankle joint. Symptoms include:

  • Limited ankle range of motion
  • Clicking or locking sensation
  • Tenderness to touch
  • Pain when flexing the ankle

Lisfranc Injury: Damage to the ligaments and/or bones of the midfoot (Lisfranc joint complex) can occur when a planted foot is twisted during a tackle, there is a sudden change of direction, or an awkward landing. Symptoms include:

  • Intense midfoot pain
  • Swelling in the arch area
  • Bruising on the bottom of the foot
  • Difficulty bearing weight or walking

Muscle Strains From Sprinting and Explosive Play

Soccer relies heavily on explosive sprinting, acceleration, and deceleration, placing significant demand on major muscle groups such as the quadriceps, groins, and hamstrings. Injuries to these muscles can affect acceleration, deceleration, lateral movement, and kicking power.

Hamstring Strains: Hamstring injuries often occur when players explode into a sprint chasing a through ball or accelerating during a counterattack, limiting their ability to accelerate. Symptoms include:

  • Sharp pain in the back of the thigh
  • Tightness during running
  • Difficulty reaching full sprint speed

Groin (Adductor) Strains: Groin strains often occur during quick direction changes or ball strikes, affecting lateral movement and kicking power. Symptoms include:

  • Inner thigh pain
  • Discomfort when cutting or shooting

Quadriceps Strains: The quadriceps muscle is heavily involved in powerful kicking motions. A strain makes striking the ball or sprinting uncomfortable. Symptoms include:

  • Pain in the front of the thigh
  • Weakness during kicking or sprinting

Head Injuries: Concussions

Although less common than lower-extremity injuries, concussions still occur in soccer. They may result from player collisions, falls, or contact while heading the ball. Symptoms may include:

  • Headache
  • Dizziness
  • Blurred vision
  • Difficulty concentrating
  • Memory Loss (transient amnesia)

Players experiencing these symptoms should be removed from play immediately and evaluated by a sports medicine specialist before returning.
