Very few help redefine an entire field of medicine.

When Southern California Orthopedic Institute (SCOI) was founded in 1976, orthopedic surgery stood at a turning point. Advances in arthroscopy, biomechanics, rehabilitation, and sports medicine were rapidly transforming patient care, yet most orthopedic surgeons were still expected to treat every condition involving the musculoskeletal system. As orthopedic science advanced, that model became increasingly difficult to sustain.

One physician believed patients deserved something better.

Dr. James Fox envisioned an organization in which orthopedic surgeons devoted their careers to mastering a specific area of the musculoskeletal system while working collaboratively as a team. Rather than asking one physician to do everything, specialists would combine focused expertise with shared knowledge, giving every patient the benefit of deeper experience and better clinical judgment. That philosophy became the foundation of SCOI.

Founded by Dr. James Fox and Dr. Martin Blazina in Van Nuys, California, SCOI grew from a small practice into one of the nation’s most respected orthopedic institutes, advancing musculoskeletal care through clinical innovation, research, physician education, and a commitment to specialized care.

Today, SCOI brings together more than 50 fellowship-trained physicians across an exceptional breadth of orthopedic subspecialties. Yet beneath every milestone, publication, fellowship graduate, and new location remains the same conviction that inspired its founders nearly 50 years ago.

A Vision That Started It All

Transformative ideas rarely begin with grand plans. More often, they begin with an observation.

After completing his fellowship at the renowned Kerlan-Jobe Orthopaedic Clinic, Dr. Fox served as an orthopedic surgeon in the United States Air Force. There, he experienced a different approach to patient care. Rather than expecting every surgeon to perform every procedure, physicians practiced within clearly defined subspecialties.

Years later, Dr. Fox recalled the realization that would shape SCOI:

“Back then, everybody was trained to do everything. At Kerlan-Jobe, everybody had done everything. But in the Air Force, I was grouped with subspecialists. I thought, ‘What a phenomenal situation for the patient.’”

As orthopedic medicine became more complex, Dr. Fox believed patients deserved specialists whose careers were devoted to mastering a specific area of the musculoskeletal system. Returning to Southern California, he partnered with Dr. Blazina to build an organization unlike most orthopedic practices of its time, one where highly specialized physicians worked together, shared knowledge, challenged one another, and delivered better care through collaboration.

The original Van Nuys office was modest, but the vision was bold. Long before multidisciplinary medicine became a hallmark of modern healthcare, SCOI demonstrated that when exceptional specialists work together, patients benefit most.

It was a philosophy that would guide the next 50 years.

From Two Physicians to an Orthopedic Institute Leader

The success of SCOI did not come from rapid expansion. It came from disciplined growth.

As patients experienced the benefits of specialized orthopedic care, referrals spread throughout Southern California. Physicians increasingly recognized that complex conditions were best treated by surgeons whose practices focused on a specific area of orthopedic medicine.

“Why should I do a hand surgery that I might perform once every six months when down the hall was a surgeon who had done it twice last week? I realized this was the way I wanted to practice my professional life. I wanted to surround myself with talent that could supplement my abilities.” —Dr. Fox

Growth followed naturally, but the culture remained unchanged. New physicians joined SCOI not only for their technical excellence but also because they embraced a shared commitment to collaboration, scientific inquiry, education, and continuous improvement. As nationally respected specialists in sports medicine, spine, shoulder and elbow, hand, joint replacement, foot and ankle, pediatrics, rehabilitation, pain management, trauma, and other disciplines came together, SCOI evolved into an integrated community of experts rather than a collection of individual practices.

Patients no longer searched simply for an orthopedic surgeon. They sought the physician whose career had been devoted to solving their particular problem. That distinction established SCOI as a destination for specialized orthopedic care and laid the foundation for decades of research, education, and clinical leadership.

The Growth of an Orthopedic Institute Across SoCal

As SCOI’s reputation grew, so did demand for its distinctive model of orthopedic care.

Each new location represented an opportunity to extend specialized expertise while preserving the collaborative culture established by its founders. Over the years, SCOI expanded to nine locations throughout Southern California, bringing nationally recognized orthopedic care closer to the communities it served while maintaining the same commitment to excellence that had defined the organization since 1976.

The timing of the expansions was significant. Southern California had become one of the world’s great centers of athletic competition. Professional sports, collegiate athletics, Olympic training, and an active population created unprecedented opportunities for innovation in sports medicine and orthopedic surgery. Advances pioneered for elite athletes soon improved care for workers, parents, students, older adults, and anyone seeking to restore mobility and quality of life.

SCOI grew alongside that transformation, helping establish the region as an internationally recognized leader in musculoskeletal care and innovation.

The Rise of Specialized Care

Looking back today, orthopedic subspecialization seems almost inevitable. In 1976, it was anything but.

SCOI recognized earlier than most that orthopedic medicine had reached a turning point. Advances in surgical techniques, implant technology, rehabilitation, and scientific knowledge demanded a greater depth of expertise. Rather than asking every surgeon to treat every condition, SCOI built an organization where physicians devoted their careers to specific areas of musculoskeletal medicine while collaborating across specialties.

The result became one of SCOI’s defining contributions to orthopedic care. Within a single practice, sports medicine specialists worked alongside spine, shoulder, hand, joint replacement, pediatric, rehabilitation, and pain management experts, allowing patients to receive care from physicians whose professional lives centered on their particular condition.

That depth became a clinical advantage. Concentrated experience drove innovation, refined surgical techniques, improved rehabilitation, and contributed to scientific discoveries that influenced orthopedic care far beyond Southern California.

Advancing Orthopedic Research and Education

From its earliest years, SCOI embraced a philosophy that continues to distinguish great medical institutions from successful medical practices: excellent physicians care for today’s patients. Great institutions improve care for tomorrow’s patients, as well.

Research, therefore, became inseparable from SCOI’s clinical mission. Every complex diagnosis, every evolving surgical technique, every rehabilitation protocol, and every patient outcome represented an opportunity to deepen understanding and improve the future of orthopedic medicine. That commitment has produced one of the most remarkable scientific legacies in private orthopedic practice.

Collectively, SCOI physicians have authored more than 1,586 peer-reviewed scientific publications and 13 orthopedic textbooks, advancing knowledge across sports medicine, shoulder surgery, arthroscopy, anterior cruciate ligament reconstruction, cartilage restoration, spine surgery, hip preservation, hand and upper extremity disorders, fracture management, pediatric orthopedics, biomechanics, rehabilitation science, and numerous areas of musculoskeletal research.

Those publications represent far more than academic accomplishment. They have helped refine surgical techniques, establish evidence-based treatment protocols, improve rehabilitation strategies, evaluate emerging technologies, and influence orthopedic care throughout the world. Physicians who have never practiced at SCOI nevertheless rely upon discoveries made by SCOI physician-scientists through journal articles, scientific meetings, textbook chapters, and continuing medical education.

For patients, the impact is tangible. The procedures performed today are more precise because surgeons challenged yesterday’s assumptions. Recovery is often faster because rehabilitation has been studied, measured, and continually refined. New technologies are adopted thoughtfully because evidence, rather than enthusiasm alone, guides clinical decision making. Scientific inquiry has never been separate from patient care at SCOI. It has always been one of its highest expressions.

A Strategic Alliance With UCLA Health

Building on the growth of the 2000s and 2010s in both the number of physicians and locations, SCOI envisioned a partnership with a group that would help bring it to new levels in Southern California. In 2019, SCOI entered an affiliation agreement with UCLA Health that would provide a partner in growth while providing stability in independence.

Together, the organizations have strengthened opportunities for fellowship education, collaborative research, complex patient care, and scientific innovation, connecting the focused expertise of a leading specialty practice with the academic resources of one of the nation’s premier health systems.

For patients, the partnership reinforces confidence that their care is informed by both exceptional clinical experience and the rigor of academic medicine. For physicians, it creates an environment where research, education, and clinical practice continually strengthen one another.

It is another example of the collaborative philosophy that has defined SCOI from the beginning.

Training the Next Generation of Fellows

Every institution leaves a legacy. The greatest institutions multiply it.

Education has long been central to SCOI’s mission. Through nationally respected fellowship programs, SCOI has helped prepare generations of orthopedic surgeons for careers devoted to highly specialized patient care. Fellows work alongside experienced physician mentors, participating in complex surgery, multidisciplinary case conferences, clinical research, and daily patient care while developing the judgment, technical skill, and professionalism that define exceptional orthopedic practice.

SCORE and the Pursuit of Innovation

Sustaining scientific discovery requires more than talented physicians. It requires an enduring commitment to research and education.

That commitment found lasting expression through the SCOI Research & Education Organization, known as SCORE. Replacing The Center for Disorders of the Knee in 1989, SCORE was established to support orthopedic research, fellowship education, professional development, and scientific collaboration. SCORE reinforces a culture in which learning never stops. Through clinical investigations, educational programs, scientific meetings, and scholarly publications, the organization continues to advance orthopedic knowledge while helping physicians deliver even better care to the patients they serve.

Like SCOI itself, SCORE exists because of a simple belief: knowledge grows when it is shared.

For nearly 50 years, SCOI has been more than an orthopedic institute. It has been an active part of the communities it serves.

From youth sports and local schools to collegiate athletics and professional teams, SCOI physicians have helped care for athletes at every level while contributing to advances in injury prevention, rehabilitation, and return-to-play protocols. Their expertise has supported not only individual competitors but also the broader evolution of sports medicine throughout Southern California.

That same commitment extends well beyond athletics.
